Application for Hospital Preceptorship Program – Fall 2016
Completed Application Packet Deadline: May 9, 2016, 5 PM, at Career Services
The Preceptorship course is a “cooperative shadowing experience in conjunction with select regional hospitals and local health providers. Students observe various medical or dental specialties under the supervision of health professionals.” Students apply in late spring of their sophomore year and would participate in the course during the fall semester of their junior year. Please note that preference may be given to students with limited health care experiences. The course will be offered in Fall 2016. Students should plan on spending 5-8 hours per week shadowing a local health provider. Students may have the opportunity to shadow one provider. Students will be required to attend a class on Wednesdays from 7-7:50 a.m. the first 5 weeks and the last 4 weeks of the semester. Please do not complete an application for this program if you are not able to attend this class time. Students will be required to pass a background check and drug screening at the hospital, attend an orientation session for the course (date and time to be announced), complete written weekly reflection assignments, and write a paper on their experiences. The course carries 2 credit hours and is graded Pass/Fail.
A paper copy of the completed application, with a handwritten signature, must be turned in at Career Services by the 5 p.m. deadline on Monday, May 9, 2016.
